What medication do you use to cope with RA? Have you experienced the effects of a medication wearing off with time? Have you tried switching to JAKi?

Hello
My wife has really bad RA. So far she has been taking Methotrexate, a DMARD. It has been working but she since she has been having more and more trouble, she thinks its efficacy is wearing off. We we thinking of switching types of medication to a JAKi (janus kinase inhibitors) but I wanted to see if anyone has had experience going from one to another or if there is a particular brand you think works well.
